Resource Planning and Management/North Campus Prescription Safety Glasses Event

When: Wednesday, January 31

9:00 AM – 1:00 PM

Where: 1602 IOE

Eligibility: EHS will provide one pair of American National Standard Institute (ANSI Z87.1-2010) approved protective prescription safety glasses to Ann Arbor campus employees every two years who meet each of the following criteria:

  • The employee is a faculty or staff member with an active, permanent appointment showing hours, effort, and compensation through the University of Michigan. Departments may obtain prescription safety glasses through this program on a recharge basis for anyone who does not meet eligibility requirements.
  • An EHS Representative reviewed the Hazard Assessment Form for the department, lab, or unit and has determined that the employee may be exposed to potential eye injury hazards, in conformance with federal and state standards for personal protective equipment.
  • The requesting department is a General Fund unit at the University of Michigan Ann Arbor Campus. Auxiliary units may obtain prescription safety glasses through this program on a recharge basis. 

How to Participate: If eligible, please read and follow the instructions below to ensure you will receive a pair of prescription safety glasses through this event.

  1. Fill out the Prescription Safety Glasses Request Form by Friday, January 26 at 3:30 PM. *When asked about “Voucher delivery method”, please select “Pick up”. This will notify EHS that you will be picking up the voucher at the event on Wednesday, January 31.
  2. Come to 1602 IOE anytime between the hours of 9:00 AM and 1:00 PM on Wednesday, January 31 to pick up your voucher, try on a variety of prescription safety glasses, and place your order. *Please remember to bring a valid prescription to place your order. Prescriptions must be in English and cannot be more than 2 years old. 
  3. Come to 1602 IOE anytime between the hours of 9:00 AM and 11:00 AM on Thursday, February 15 to pick up your prescription safety glasses.

 

*There are upgrade options available. Any upgrades are the responsibility of the employee alone and must be pre-paid at the event via check or credit card. Cash will not be accepted for security reasons.
